PHOTO / VIDEO RELEASE

I hereby grant to S.I.N. 360 (formerly S.I.N. Dental USA) full and complete rights to the use of my image and voice (still photograph, audio or video recording), with or without the use of my name.

I understand that my image may be edited, copied, exhibited, published or distributed and waive the right to inspect or approve the finished product wherein my likeness appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of my image or recording. I also understand that this material may be used in diverse educational settings within an unrestricted geographic area. Photographic, audio or video recordings may be used for the following purposes:

• Digital Advertisement / Marketing materials for S.I.N. 360 and/or any events, exhibitions, or shows sponsored by S.I.N. 360, including but not limited to display ads, video ads, social media posts, and promotional materials.
• Educational or informational presentations, courses, promotion or marketing materials

This release is given without charge to or any remuneration from S.I.N. 360.

I hereby release and agree to hold S.I.N. 360 free and harmless from any claims of copyright, libel, slander, invasion or violation of privacy or other similar rights that I may hold or assert.
